Upon creation of your appointment you will be e-mailed instructions to fill out all of your first visit paperwork.
If you prefer, or if you do not have an e-mail address, you can fill the forms out on one of our kiosks at the time of your first appointment. In this case, please arrive 30 minutes early. Insurance information in the form of a card or a claim information form (when using auto, accident, or workers compensation insurance) is also needed.
Please dress comfortably as your therapist will be performing a
clinical evaluation to assess your problem and establish the best treatment plan. Skirts or tight fitting
jeans are not recommended.
Provided that all of your insurance information is complete and accurate, we will bill your insurance as a courtesy. Co-payments are expected at the time of service.
