For your convenience, you can download these forms to read and complete prior to your first visit. If you prefer, you can fill them out at the time of your first appointment and in this case, please arrive 15 to 30 minutes early. Insurance information in the form of a card or a claim information form (when using auto, accident, or workers compensation insurance) is also needed.
Please dress comfortably as your therapist will be performing a
clinical evaluation to assess your problem and establish the best treatment plan. Skirts or tight fitting
jeans are not recommended.
Provided that all of your insurance information is complete and accurate, we will bill your insurance as a courtesy. Co-payments are expected at the time of service.
